If you are looking for a family physician, we welcome you! There are several forms that must be completed before your first visit; please download these forms, fill them out completely, and bring them with you to your first appointment. If you have any questions about them, you may as the receptionist when you arrive. For your first appointment, please arrive at least 15 minutes early so that we can process your papers.
If you have trouble viewing these forms or are unable to print them and fill them out before your appointment, please come 30 minutes early to your appointment and you can fill them out in the waiting room. Thank you, and we look forward to seeing you!
